Annual Open Enrollment is your opportunity to learn about the 2026 Health Care Benefits offered through USEFCU, review your current coverage and choose the best options for you and your family.
We’re excited to share that there will be no changes to our carriers and only minor updates to the HMO medical plan. Even better—medical premiums will decrease in 2026!

ENROLL ONLINE
Please Visit www.paycomonline.com by Monday, November 24th!
Everyone is encouraged to view and update their benefits through Paycom between November 17th and 24th.
Benefit changes outside of Open Enrollment require a Qualifying Life Event and/or Loss of Eligibility.
You may not change your benefits throughout the year unless you experience a Qualifying Life Event. Qualifying Events may include marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you must notify HR within 30 days of the event; you will then be able to log in to Paycom and make changes to your benefits. Please remember you only have 30 days from the event to make changes to your benefit elections.

Medical plan highlights
- Overall medical premium cost is going down in 2026.
- You may access your new ID card(s) through myuhc.com and the UnitedHealthcare app
Pharmacy Copays for Both Plans
HRA
Health Reimbursement Account
- Automatically enrolled when you enroll in medical coverage
- USEFCU funds members HRA with $1,000 each year
- Members can be reimbursed medical deductible out-of-pocket expenses
- Explanation of Benefits (EOB) must be submitted
- EOBs can be downloaded through your login at myuhc.com
- Unused funds do not carry over to next year
DENTAL
VISION
Vision Plan Highlights
- Routine Eye Exam - Every 12 Months $10 Copay
- Eyeglass Lenses - Every 12 Months $25 Copay
- Eyeglass Frames - Every 24 Months $25 Copay
- $130 Allowance + 20% remaining balance
- Contact Lenses and Fitting - Every 12 Months $60 Copay (fitting & Evaluation)
- $130 Allowance
BASIC LIFE/AD&D
- Benefit amount is 2x Annual Salary up to $200,000
- Automatically provided by USEFCU to all full-time employees
Beneficiaries can be updated at any time during the year through Paycom.
SUPPLEMENTAL LIFE/ AD&D
Employee Life and AD&D:
- $10,000 increments up to 5x annual salary ($300,000 Maximum)
- Amounts over $50,000 will require an Evidence of Insurability (EOI) form for approval
- Can increase by $20,000 each open enrollment without EOI
- You must elect employee life in order to enroll spouse or child(ren)
Spouse Life and AD&D:
- $5,000 increments up to $50,000 (not to exceed 50% of employee election)
- Amounts over $25,000 will require an EOI form for approval
- Can increase by $10,000 each open enrollment without EOI
Child(ren) Life and AD&D:
- $10,000 Benefit for children 6 months to 19 years (25 years if full-time student)
- $1.86 per month (one rate covers all dependent children)
Voluntary Life monthly premium is calculated by benefit amount elected and employee's age.
Cost is automatically calculated when you ernoll through Paycom.
DISABILITY
These benefits are provided at no cost to all full-time employees.

Employee Assistance Program (EAP) Service Include:
- 24/7 unlimited telephone access to EAP professionals
- Confidential Free Assessment / Referral / Follow-Up Services
- Face-to-Face Assessment Interview
- Master-Level Licensed Counselors
- Referrals to Community-based and / or Benefit Covered Resources
Benefits to employees:
- EAP is a benefit available at no cost to the employee and immediate dependents
- EAP is an objective resource available to assist in problem identification and resolutions
- EAP is confidential for all individuals who self-refer
